Question 1:  How do you get the inside bulbs out?  They're up against the reel body.  Do you take lamp assemble off reel? If so how?   

Question 2:  What #bulb does the reel use?  What #bulb for door button?

the machine I have had 658 in door buttons and 359 on reels.

Itake the reel strip off or just get the lamp holder out with a needle nose.

I'll give the needle nose pliers a try, I'd rather not take the reel strips off.  How does the entire lamp holder come off?

The bulb numbers you have are correct. I guess you can take the reel off but that is not ideal and you can not really take the lamp box out that easily.  Usually just take the reel out and turn it on its side and take the lamp holder out. I have done it with no issues and just takes time etc.

359 is a xeon bulb and only used for the backlit reels
658 is for the button deck and used on all S2000

I don't like removing the reels strips either.  There is one Phillips screw that holds the light assembly on. 
Unplug the light assembly three plugs; remove the Phillips screw that holds it to the reel assembly and now you can remove the whole light assembly. 
Now replace your bad bulb or bulbs , reinstalled on reel assembly and you are really to go.
