is an opto isolator simply good or bad or can they be intermittent or pass an inaccurate signal?

It depends on what testers and why you are testing it? Circuit boards have no end these days...

Where specifically is this optic isolator located and on what kind of machine?
Maybe we can help you more.

A general answer to your question would be "yes, they can go bad"...but
that would be my same reply to any other machine part too... :72-

In all my years working on TVs I never came across anything but good or bad ones. Never saw one that was damaged but partially working. The ones I dealt with were in the power supply section. What are you dealing with and why do you suspect it's faulty?

this is on my cei slot.  The reel speed and position is detected by an optical sensor that feeds into an opt isolator.  The manual was  saying to replace the opto isolator but I did not believe it could be intermitent.   I have it working again and probably caused the issue myself.  Between adjusting the optical sensor air gap and forcing a reel calibration its back in action.

What are physical slot steppers?  Hall affect switch with a slot in the reel?    These have a sensor with an led that shines against a series of wide and narrow reflective strips on the reel edge.  The reflection is picked up by an optical transistor that transmits back to the main board and the first step is an opto isolator.

What are physical slot steppers?  

In the slot machine world...these are otherwise called...uh... "reels"...
Another term is>> "Stepper Reels"
They are controlled by the notches on an internal wheel passing through optics.
The wide/narrow reflective strips on yours is an upgrade (somewhat) to the older "notches"
for controlling the reels.

Optos do indeed degrade. Had that issue with centipede and Tempest and back when we were designing Arkanoid all the time. they degrade, get less sensitive and need replaceent. Regapping doesnt help out., Maybe the emitting LED dims like LEDs do after time...
