Looks to be from "Bally Slot Machines" by Marshall Fey, published by Liberty Belle Books. But I may be wrong. Here's one for sale on fleabay. It's half the size of a regular 8 1/2 x 11 and it's black and white. The resolution is so-so, I can't make out the pay scale's on the glass. It's from Liberty Belle Books. Did this book come out in color and more of a full size ? Still a nice reference.
