I don't know about a "worst" slot machine, but I think the bonus round on the Double Diamond Run vision machine is pretty cheesy

I remembered seeing one of them on a casino floor once.  White glass with black text, white reel strips with black printing.  I've never been able to find a picture online because if you search for "Slot Machine" (which was the name of the game), it obviously brings up eight million results that aren't what you're looking for.
