For tackyness and ugly factor my vote goes to IGT's "generic slot" machine. A clone of Wild Cherry this game was black and white and the belly glass said SLOT MACHINE.

Wow really?  Was it an S2000 or an S+?  Any photos of said machine?  Were they abundant on the casino floor at any time?

I agree that the one pictured is a Five Times Pay clone.  Of note, it looks like there's a logo that isn't IGT's logo in the corner of the glass.  It's too small to tell, but perhaps it's Sunkist Graphics?  Point being, I don't think we can blame IGT for creating that game.

Not sure, but I believe that "logo" might be a fake bar code giving the machine that "generic" look.
But I am positive the one I saw at the Colorado Belle was a Wild Cherry clone. So there are more than game type. I had a pic of it but it was lost when the hard drive it was on failed.

Your right, not an IGT logo. Hard to make out, Sunkist would be my first guess too.
