I have a Dell Inspiron and I'd like to connect to the internet using my iphone5 as a hot spot.  The Dell desktop is at my shop and I do not have an internet service.  When i turn hotspot on on the iphone it does ask me if "I trust this computer" but I can't figure out how to connect.

Are you connecting wireless or through USB?  If you are only connecting 1 PC, you are better off tethering using the USB port, it will charge your phone while you use the tethering.  Wireless tethering eats up your battery really fast. The procedure is the same, you enable personal hotspot, and select USB.  You will need to have the latest version of iTunes in your pc for it to work.

I don't have itunes on the computer. How can i install itunes w/o internet connection?

Download the itunes install file on a pc with internet, copy to a usb drive and install in other computer with it.

I downloaded file and installed on the dell desktop computer.  I turned on hotspot on my iphone, then connected the cable from phone to usb port.  There is a "bing" and the message on my cell phones asking if I trust this computer, I tap the "ok" and message goes away. 
Problem the network/iphone does not show up.

shouldn't the iphone show up?

What version of windows is in the old PC?  The iphone should show up as an additional network adapter.

when you plug your iphone, does it show up as a network adapter in your device list?  Sometimes, you got to give it time for windows to install the network drivers for the iphone.
