I had never heard of it or seen it before.  I can't help about it being EPROM-based or not, sorry.  For anyone else who hasn't seen it before, here's the description from IGT's site:

For nostalgia value alone, Swing is the hap'nin thing for the next millennium. Join the action when the rockin' Big Band Swing sounds of Louie Prima and Indigo Swingâ„¢ Video Slots get your hip-hoppin' at the slots.

This lively, upbeat game creates a Swing Club atmosphere wherever it's played. Toe-tappin' music accompanies each reel spin, punctuated by an accent of jazz as each of the five reels stop.

The Swing party really gets rollin' when three dance couples line up on any of the nine paylines, initiating the bonus mode.

LASlots.com lists it on their website as a "I Games[sic] Plus" machine and their picture, which just appears to be a blown-up version of your picture, does show the second row of buttons on the deck.

I found the company that did some of the music for the machine -- they say that they made the music in 1998.

From a 1999 IGT press release:

maybe , blueridge, joey, rocky mountian, someone has had to have some of these come in there shop, i have played before very cool game ;
but for some reason the casino would not let me look inside

That looks like a fun game. I almost remember playing it in the casinos along time ago.

according to IGT's website this is the program group and it is definitely eeproms not flash.  i would send you the PSR and PAR but i can't e-mail them from work.  hope this helps!
