My boy put two coins in the machine and once I got the coins out it seems that about 50% of the time the coins don't reginster and count the credit.  Is this something I need to adjust?  If so how do I adjust it?  Also, should I try a different coin?

I have an IGT S+

It sounds like the coin-in optics are not always "seeing" the coins dropped.
It's possible that the plastic guides inside the coin-in optics are broken or missing.
That would cause the optical eyes to misread the coins.

Just to verify, are the coins that are not getting counted going to the hopper or back out the machine to the coin tray?  If they are going to the hopper, I would agree with stayouttadabunker and it's something with the optics.

Then it's not the optics, it's the coin acceptor.

2 things.  First, I have found using an older style quarter - the one with the eagle on the back - seems to works best as a sample coin.  It seems with all the different quarters out there now, it can be a little tricky to get a sample coin that works with everything.  Second, if your coin acceptor is like the one in the picture below - there is a adjustment knob (I've circled it in red) to control the sensitivity of the acceptor.  Try turning this all the way down counterclockwise.  It may help...

Here is a picture of mine.  Good idea, I will try another coin.  Mine doesn't seem to have an adjustor.  It worked great before this issue.  any other idea's?

That's the wrong acceptor to use for quarters.  That one is made for large denomation coins.  I'm not really sure what the technical reason is that it doesn't work for quarters - but I have seen that issue before.  Can anyone else chime in on that?

Thanks I tried it but seems about the same.  I would say every 8th quarter or so.  Should I buy a new one?  Is that my fix?  Any other idea's?  By the way... thanks for helping, this place is awesome.  Obviously I'm a newbie

Because it now happens every 8th quarter or so, I am tending to believe that something inside the coin mech is going bad as the electronics inside begin to heat up....is the cover really very warm or hot as you touch it?
Leave the machine on for a couple of hours and good try and insert some coins and check the temperature of the coin mech cover.
If it is very warm, then one of the electronic components on the board may be going or a solder joint of one the components is loose...thereby creating heat and causing irregular sensor readings which in turn causes the coin mech to misread the coins going through and then spitting them back into the tray.
If the coin mech is really warm, I would dismantle it (take the cover off) and peer around inside and try to find a loose connection such as a resistor or something. You may find some parts of the board look like they were overheated when you hold up the board with a strong light behind it. Get a good magnifying glass and really look good at every piece of wire soldered onto that board..if you're good with a soldering iron, you may be able to remelt the solder on the bad joint to get a better connection to the board.
Finally, try shopping around for a good spare...there are many vendors located right here on NLG's home page or post up that you want one on the classified section of this site....I'm am very sure you can get one pretty cheap.

Is there a 3 wire harness connection to the coin acceptor? It would connected to the board labeled 09270250-R2.

Yes it has a threee wire harness.  Yellow, Purple and black wires.
