I had never heard of it or seen it before.  I can't help about it being EPROM-based or not, sorry.  For anyone else who hasn't seen it before, here's the description from IGT's site:

LASlots.com lists it on their website as a "I Games[sic] Plus" machine and their picture, which just appears to be a blown-up version of your picture, does show the second row of buttons on the deck.

I found the company that did some of the music for the machine -- they say that they made the music in 1998.

From a 1999 IGT press release:

maybe , blueridge, joey, rocky mountian, someone has had to have some of these come in there shop, i have played before very cool game ;

according to IGT's website this is the program group and it is definitely eeproms not flash.  i would send you the PSR and PAR but i can't e-mail them from work.  hope this helps!
