It kinda looks like a variant of the Cascade of Diamonds machine, which used an IGT S+ as its base, along with a third-party top box for the bonus. See the description and pictures here:

Edited to add: good find, Fourbyracer! I had forgotten about that thread.

To the original poster: support and software for that topbox will be nearly non-existent, so if it's not working I'd steer clear of it unless you have a good idea of what's wrong with it.
