  I  remember hearing that exposing that window on the chips to light is one way to erase it. Cover the window with a label or piece of electrical tape and try to write to it again.  Dont know if it will help but it may be worth a try until someone with a better understanding sees your post.

I believe the GQ-4X does not need adapters to do a 40 DIP, post a question in MCUMALL forum about that chip and programmer, specifically.
They will usually answer quickly.

It depends upon if the chip is 8 bit or 16 bit. Most of the regular eproms are 8bit, but the ones that we use for the base and key chips (27C4096 or 27c4002) are 16bit eproms, and they DO require and adapter for the GQ-4X. The adpater that I use was from the MCUMALL Folks, but i am not sure about the adapter pictured. I agree that he should post a question with the picture of the adapter on that forum.

I posted a question about the 4002/4096 on there.
It has me curious. I would love not having to run Sun/Oracle VirtualBox to do slot stuff.
my EasyPRO 90B does them without adapters, but it is limited to a 32 bit OS for now, until the low level drivers are done for 64 bit as well.

Munch, you probably have done all of this already, but I thought I'd add my two cents just in case. Tongue Out wave

You said that it doesn't write, but does it read?
Have you been able to confirm that you've inserted the adapter correctly? (I see exposed pin holes in the GQ-4X. Does the adapter use only 32 pins?) 
Are you sure that you've selected the right chip in the software?
If the right chip is selected and the 4X software indicates that you should have the adapter inserted, etc., can you then successfully read the ID of the chip?
The jumper looks like it is set correctly. Have you tried the chip in the other socket as Tilt suggested? Just for giggles, have you tried changing the jumper anyway?

I have good news and bad news.
 The bad news is the adapter from China doesn't work.   I don't know if it's just incompatable or broken - the result is the same it doesn't work with my programmer. 
The good news is I got the "official" adapter from mcumall and it works perfectly. 

It only took 4 days to get to me too.   Mail

I'm on my iPhone so I can't look up the
price but there were two reasons

The first was price I think $10 cheaper
the second and deciding factor
was mcu was on backorder - remember I started
this a month ago.

nope- once I got the adapter - it was able to read and burn easily.  The cheap adapter on eBay didn't work.  The one from mpumall did.
