I have a PE Multi Poker that the hold buttons don't work during game play but work in test mode, also the menu button won't bring up the menu (uses one of the hold buttons). The only anomaly I see is that the indicator for door closed is flickering.  The machine was intermittent, now the hold buttons don't work at all.  I cleaned the board connectors to no avail.  Otherwise the game coins up and plays normally.

I think that the buttons are wired in series, check the connections to each button, if one is bad, all will go out. I know that you said you tested but just check for giggles.

If the machine isn't seeing the door as closed, the buttons won't work to hold cards (in the middle of a hand), or to access the menu.

With the "indicator for door closed" that is flickering, are you saying that the machine is alternating between displaying a "Door Open" message, and a "Closure" message, or are you saying that in the test page with the door closed, the test alternates between 1 and 0 rapidly (which is normal)?

The hold buttons are wired in series, but the far left hold button isn't in the same series as the rest of them (if memory serves -- I'm not in front of my machine at the moment).  Do all five hold buttons fail to work in the middle of a game, or just some of them?  Do all five buttons work in the self test page, even with the door closed?  (I'm thinking that perhaps a wire may be getting pinched after you close the door, causing them to work with the door open, but not closed.)
